Web-Based Information Management of Maintenance Errors in Aviation Mishaps
نویسندگان
چکیده
This paper describes a safety information management system designed to capture maintenance factors that contribute to aircraft mishaps. The Human Factors Analysis and Classification System-Maintenance Extension taxonomy (HFACS-ME), an effective framework for classifying and analyzing the presence of maintenance errors that lead to mishaps, incidents, and personal injuries, is the theoretical foundation. An existing desktop mishap application is updated, a prototype web-based model is developed, data-mining techniques are discussed and an Asynchronous Distributed Learning (ADL) module is conceptualized. These tools facilitate data collection, organization, query, analysis, and the reporting of maintenance errors that contribute to aviation mishaps. Together they represent a complete, robust system for analyzing aircraft maintenance mishap related factors anywhere at anytime.

A comparison of leading and lagging indicators of safety in naval aviation.
BACKGROUND The purpose of this paper is to examine the results of two different methods of identifying human factors safety concerns in U.S. Naval aviation. In both studies, the information was collected using the Department of Defense Human Factors Analysis and Classification System (DoD-HFACS). METHODS In the first study, aviation mishap data (a lagging indictor) was obtained on 47 F/A-18 a...
متن کاملDevelopment of a Knowledge Management System to Reduce Errors in Aviation Maintenance
One of the causes of aviation accidents is lack of overview and analysis of the data particularly obtained from its maintenance operation performance. The aircraft maintenance system of an airline is a complex structure with information flow involving various stakeholders ensuring aircraft airworthiness while adhering to several regulatory standards, policies and procedures of Federal Aviation ...
متن کاملHow System Errors Affect Aircrew Resource Management (CRM)
System errors, both mechanical and human in nature, can have a grave effect on aircrew judgement in flight. The effects of these errors can be massively compounded during emergency situations. Crew Resource Management (CRM) is an important process aircrews can utilize to minimize risks and enhance assessments. The employment of this technique can be validated by aviation mishaps over the last t...
متن کاملA Strategy for the Development of a Web-based Tool to Reduce Aviation Maintenance Errors
The safety and reliability of air transportation depends on minimizing inspection and maintenance errors that occur in the aircraft maintenance system. Efforts have been invested to track maintenance errors. These efforts are reactive in nature: they analyze maintenance errors after their occurrence. There is a lack of standardization in the assessment of maintenance errors across the maintenan...
متن کاملSelection Strategy for Identification of Process Measures for Surveillance in Aviation
Inspection and maintenance errors that occur in aircraft maintenance systems have a formidable impact on the safety and reliability of air transportation. Evaluation of the aircraft maintenance system requires an analysis of the maintenance processes in use. The systematic evaluation of data collected on the aviation maintenance process can provide management with feedback on the performance of...
